COMMUNITY NURSE

Location: Mid Essex

Salary: £30,000 - £40,000

Role and Responsibilities: As a Community Nurse in our hospice, you will play a pivotal role in delivering high-quality palliative and end-of-life care to patients within their own homes. You will collaborate with a multidisciplinary team to create and implement care plans tailored to each patient's unique needs. Your responsibilities will include:

  • Providing compassionate nursing care, symptom management, and pain relief to patients in their homes.

  • Offering emotional support and guidance to patients and their families, helping them navigate this challenging time.

  • Collaborating with other healthcare professionals to ensure seamless coordination of care and holistic support.

  • Educating patients and families about their condition, treatment options, and available resources.

  • Contributing to the ongoing improvement of our services through feedback and collaboration with the team.

  • Proficient in using electronic medical record systems.

Qualifications and Requirements:

  • Registered Nurse (RN) with a valid and current nursing license.

  • Experience in palliative or end-of-life care is preferred but not essential – a strong passion for this field is crucial.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and as part of a collaborative team.

  • Empathy, compassion, and a commitment to delivering patient-centred care.
